We participate in most insurance plans including Medicare. For plans requiring a co-pay or for cosmetic procedures, payment is required at the time of service. Co-payments and co-insurance are determined by your insurance company. Please click here for a complete list of participating insurance providers (need link to Participating Insurance Providers). Please contact your insurance company directly with any questions you may have regarding your coverage as plans may vary.
If your insurance company requires a referral from your primary care physician, please contact this physician prior to your visit. We are happy to supply relevant information you may need, but it is your responsibility to obtain the referral. If a valid referral is not on file at the time of your visit, you will be asked to sign a referral waiver on the date of the appointment or given the option of re-scheduling your appointment.
Missed Appointments and Cancellations
Missed appointments or short notice cancellations make it hard for us to offer other patients timely appointments. Please give us at least 24 hours notice if you must cancel your appointment. Missed appointments or providing us with less than 24 hours notice for a cancellation is subject to a $40 fee.
Policy on Relationships With Industry Representatives
While our practice accepts educational materials and medication samples often provided by pharmaceutical industry representatives, we cannot accept any meals, drinks, or other elements of value not directly related to patient care. We strive to provide outstanding medical care driven by our patients’ well-being first and foremost.
